Scope and Contents

This series consists of original and photocopied Joint Hospital Data Questionnaire forms and the various versions used to collect statistical information on hospitals throughout Georgia. This data helps provide hospital care health planning and resource development for the state. Information included on the forms is as follows: (1) reporting period (2) hospital name, complete address and phone number (3) name and phone number of person to call for further information (4) type of organization (state/local/federal) - non government (IRS tax exempt) (5) type of services provided (6) whether admissions are restricted primarily to children (7) whether accredited by Joint Commission on Accreditation of hospitals (8) whether approved for Vendor Payments (9) bed capacity/licensed/currently set up and staffed - patient census (10) whether permanent or significant temporary change in beds set up and staff to show increase or decrease (11) types of service and levels of care by beds set up and staffed (12) outpatient services (13) number of visits made to outpatient services (14) medical and other staff coverage (15) hours of operation (16) services/facilities breakdown within each service showing whether within hospital, shared or contracted workload totals (17) personnel on payroll breakdown by service and by position and whether full or part-time (18) hours worked last week of report period by part-time employees and totals (19) medical staff and affiliates showing total active staff and affiliates (20) training and education programs offered by the hospital or through agreement with educational institutions or training centers (21) financial data (listed by source) (22) expenses (23) long term debt and repayment responsibility (23) whether hospital currently uses electronic data processing equipment and what type program format is in use. The State Health Planning and Development Agency has the major responsibility for providing planning and technical assistance to managers and planners in meeting the expectations of the federally funded comprehensive health planning programs.
