This series contains documents relating to child health services provided to children in each county in Georgia. Among these documents is form DPH/HIS (2) - 10 (Rev. 7-71) entitled Child Health Services Report. This form is submitted to the Child Health Unit monthly by every county and contains the following health care information arranged by age and race of child: (1) children seen for first time; (2) children seen for first time for a specific year; (3) number of home visits; (4) number of children served in homes for first time for a specific year; (5) medical examinations; (6) complete and partial nursing appraisals; (7) hemoglobin counts; (8) iron medication given; (9) referrals. Also included is the annual report summarizing the monthly reports from the counties.
