Education - Office of Instructional Services - Teacher Certification Name Index Cards
Scope and Contents
This series contains at least two different types of index cards. The first type indicates for each certified teacher in Georgia: (1) name, (2) address, (3) sent to (unclear as to significance ‑ when completed a college name or date of birth is indicated), (4) number of application, (5) number of certificate, (6) kind of certificate, (7) date issued. The second type of index card, in addition to furnishing the above, provides blanks for: (8) date of birth, (9) place of birth, (10) sex, (11) race, (12) father's name, (13) mother's maiden name, (14) total credit, and (15) field. Unfortunately, most of these additional blanks were never filled in by the Department of Education. Many of the cards are stamped with a rubber stamp indicating an apparent conversion from an old application and certification system to a new one. Old application and certification numbers are indicated in the stamped blanks.
