These visiting teachers' forms vary somewhat from county to county and month to month, but all generally contain the following enrollment data divided into White and Black totals: (1) number of schools (2) total school age children residing in school system (3) total school enrollment each month, last day of month and average daily attendance in the public schools in the county (4) the percent of daily attendance in the public schools (5) the same information (3 & 4) for private and denominational schools, and (6) total figures for both public and denominational and private schools. Visiting Teacher Activities data include: (1) number of visits to schools (2) number of conferences with school personnel (3) number of interviews with individual pupils (4) number of active cases per month (5) number of contacts with active cases; visits; others (6) number of children entering school for first time in year; reenter (7) number of families referred to other agencies for assistance (8) number of contacts with landlords, employers and other parties concerned (9) number of meetings attended where pupil welfare discussed (10) number of cases referred to juvenile court (11) number of parents or guardians brought to court (12) number of convictions in court (13) number of children placed in homes or institutions by court, and (14) number of days spent in office; field. The forms are signed by the visiting teacher and the superintendent.
