This series contains correspondence concerning certification of midwives in Georgia and forms listing the names of all midwives seeking annual certification in Georgia, retiring or who passed away during the specific year covered. All deliveries attended by each midwife are also listed by year. Accompanying this first form is a brief statistical summary called an Annual Certification Action Record. In addition to providing much the same information as the first form, the names of midwives not applying for certification (but still not officially retiring and those moved from the county during each year are also provided. All of these records were accumulated by the Director of the Maternal and Child Health Services and continued by the Maternal and Child Health Office of the Physical Health Division of the Department of Human Resources after the government reorganization of 1972.
