Scope and Contents
This series consists of surveys initated and newspaper clipping collected by the Georgia Dental Health Program in the 1940s through 1960s. Thomas L. Hagan, the Senior Dental Surgeon of the U.S. Public Health Servicere created dental health survey reports from varying Georgia. John E. Chrietzberg, Director of Dental Public Health Services of the Department of Public Health conducted surveys from 1958-1964 through questionnaires sent to Mental Institutions, Hospitals, Care Facilities, and Colleges in Georgia to acquire data regarding the needs of dental care in these facilities. This series contains: (1) Reports of Dental Surveys conducted in Georgia in 1946-1947; (2) Dental Resources, Programs and Needs in Institutions in Georgia; (A) Questionnaire(s) on Dental Personnel and Salary Needs in Mental Institutions-Out of State; (B) Survey(s) of Dental Resources, Programs and Needs in Institutions of Education with students in residence; (C) Survey of Dental Resources, Programs and Needs in Hospitals; (D) Survey of Dental Resources, Programs and Needs in Nursing Homes, Homes for Children and Institutions for the care of the chronically ill and the aged; (3) Questionnaires which include the following sections: (A) Name of Institution; (B) Name of City; (C) Name of County; (D) Types of dental services offered by the institution; (E) Financial support source; (F) Available equipment for dental care; (G) Size of patient/student body in the institution; (4) Survey Summary and Reports; (A) Dental Resources, Programs and Needs in Institutions in Georgia; (B) Georgia State Plan for hospitals and related facilities; (5) Dental Health Conference Reports. Series also includes newspaper clippings from 1951-1969. Survey reports listed patients as child or adult and as white or not-white. These survey reports were strictly between 1962-1963 gathering and accumulating data by county to provide state total records.

Biographical / Historical
The Georgia Department of Health was initially enacted in 1875 to aid in resolving inquiries with respect to causes of diseases, investigate sources of mortality and the effects of localities, employment, and other conditions upon public health, to supervise the registration of births, marriages, and deaths, etc. On Aug. 17, 1903, another act was passed, reinstating the department a second time. The Georgia Dental Association was founded in 1859. The organization was reorganized on July 30, 1869, following the reorganization of the Southern Dental Association. The Association now boasts a membership of over 3500 dentists dedicated to improving dental health in Georgia.
John E. Chrietzburg, DDS served as Chief of the Illinois Division of Public Health Dentistry from 1947 – 1953. He served as the Director of the Georgia Dental Health Branch of the Georgia Department of Health from 1953 – 1967.
