This series consists of (1) applications for participation, (2) survey report forms, (3) status of application, (4) plans for correction of deficiencies, (5) utilization review plans, (6) transfer agreements, (7) quarterly personnel reports, (8) Title VI clearance, (9) reports on Civil Rights inspections and (10) related correspondence. These files hold the information that determines whether or not a hospital or an intermediate care facility meets or continues to meet State and Federal requirements for participation in Medicaid and Medicare Programs (Titles XIX and VI, respectively - Civil Rights Laws). It is the responsibility of the Standards and Licensure Section to determine the adequacy of facilities and services, through investigation, inspection and evaluation, in relation to requirements for the licensure and certification of hospitals, nursing homes, home health agencies, portable x-ray services and rehabilitative agencies for this participation.
