Scope and Contents

This series contains a collection of forms employed by county officials to report to the original Georgia Department of Public Health births, marriages and deaths occurring in their respective counties during the years 1875 and 1876. This program, far ahead of its time, only existed for two years and then was abolished until the 1930's. Although statewide in scope, only the few forms listed in the shelf list below survive. Three forms were apparently utilized. Form A is a letter size sheet designed to be returned to the appropriate county ordinary by the physician attending a birth. Each form indicates: (1) Name of county (2) Date of birth (3) Full name of child, if living (4) Sex and number of child of mother (5) Color (6) Place of birth (city or country) (7) If premature, period of utero-gestation (8) Cause of miscarriage (9) Condition of mother subsequent to miscarriage (10) Full name of mother (maiden name) (11) Mother's birthplace and age (12) Mother's residence (13) Full name of father (if illegitimate, so state) (14) Father's birthplace and age (15) Certification by the physician of the true facts of the birth repeating names of the mother and child, the date of birth and signed by said physician. Much more numerous and apparently the correct form designed to be forwarded to the State Department of Public Health by the county ordinary is Form C. For births, each form indicates the identical information requested on Form A and indicated above: The only difference in the two forms is, of course, the official who completed them. Form C recording a death indicates: who completed them. Form C recording a death indicates: (1) Name of county (2) Full name of deceased (3) If unnamed, name of parents (4) Date of death (5) Age (6) Color (7) Condition (marital status) (8) Occupation (9) Birthplace (state or country) (10) If foreigner, how long in locality (11) Father's birthplace (12) Mother's birthplace (13) Place of death (city or country) (14) Cause of death (primary and secondary) (15) Recording date (16) Signature of county ordinary. Form C recording a marriage indicates: (1) Name of county (2) Full name of groom (3) Residence (city or country) (4) Age (5) Color (6) Occupation (7) Place of birth (state or country) (8) Father's name (9) Mother's maiden name (10) Identical information for the bride except an indication of maiden name in the case of a widow remarrying (11) Name and title of official performing the ceremony (12) Recording date (13) Signature of county ordinary. Finally, this series contains a few forms designed to record totals of births, marriages or deaths for two month periods. These forms are not usually completed in full but are signed by the appropriate county ordinary.

A complete list of counties and categories extant is in the finding aid, but it cannot be construed that all forms for a specific county and category are present.
